Job Description

Saint Alphonsus Medical Center is looking for a Family Practice physician for their Nampa location to start as soon as credentialed!

• Location: 12th Ave Family Practice
• Schedule Needed: M – F, 8a – 5p, every other week
• Practice Setting: Clinic
• ~18 ppd
• 3 physicians in practice
• 15 exam rooms
• EMR: Epic/Cerner
• Trauma Level/Designation I
• Patient Population: OB, Pediatric, Adult, Geriatric

Submission Requirements
• Board Certified – Required
• Active ID license/IMLC – Required
• DEA/CSR – Required
• Clean malpractice and no background – HIGHLY PREFERRED
• Local candidates, no flights – HIGHLY PREFERRED

• **Must have COVID vaccine in order to work at this facility – Required**

If provider does not have active license at the time of submission, they must submit license application within 3 days of acceptance.*

Certification Requirements
• Board Certified
• Controlled Substance (CSR/CDS)
• DEA

State License Requirements
• Idaho
