facility is seeking a urologist to assist with clinical coverage,
offering clinic-only hours from 7am–5pm for adult patients only,
starting with sporadic dates and aiming for more consistent coverage
over time. The provider will see 2–5 patients per day, perform general
urology procedures, and work within the EPIC EMR system. Candidates must
be board certified or board eligible within five years of residency,
hold an active Indiana license at the time of submission, and possess
ACLS certification. Credentialing timelines include 90–120 days for
payer enrollment (upon receipt of demographics, IN license, IN CSR, DEA
with IN practice address, and signed forms) and 45–60 days for hospital
privileges after submission of a complete CVO/CPI Portal application.
