Job Description
Blanchard Valley Health System is looking for a Family Medicine provider to start ASAC! Willing to consider locum to perm for the right fit.
• Cerner training completed prior to start
• Orientation
• Staff
• Office
Schedule:
2-5 days per week
o No weekend clinic hours- Procedures are scheduled during clinic hours.
Types of procedures:
o Liaisons
o Wart removals
o EKG
o Nail removal
o Abscess
Patients:
• Average number of patients per day: 20-25
• Peds: Yes
• Types of Cases:
o ADHD
o Well baby
o Acute
Submission Requirements
• Board Certified/Board Eligible – Required (BE within 5 years of residency)
• Active OH license – Highly Preferred
• Clean Malpractice – Highly Preferred but can consider providers with case payouts, can not have actions on licensure
• COVID vaccine – Required (submit with name clear please)
The following MUST be included in the presentation:
• Why is the provider leaving their current workplace?
• Have they ever been terminated?
• Why are they doing locums? Or wanting to do locums?
• Are they vaccinated?
• Are they open to locum to perm?
