• Schedule Details: 2 weeks on/2 weeks off - Clinic hours: 8 AM - 4 PM
• Number of Shifts Needed per Month: As per schedule
• Procedures: Patient assessments, new treatment plans, ongoing treatment plans, changes in therapy, referral for ancillary services, end of life care, chemotherapy management, pain management
• Certifications Needed: Relevant certifications in Hematology/Oncology
• Patient Population/Case Load: Clinic Provider Daily Volume: 10-30; Average Daily Hospital Practice Census: 10-15; Patient Population: Primary Clinic - Medicare/Medicaid/Commercial; Secondary Clinical - Commercial, Medicare
• Case Mix: Inpatient Responsibilities: Yes, Inpatient coverage of both sites
• Telehealth: Yes, there may be telehealth in addition to inpatient responsibilities
• Appointment Length: Consult - 40 minutes, Follow up - 20 minutes

• License: An active Iowa state license or IMLC membership is required
• Willing to License: No
• Estimated Privileging Timeline: 60-90 days
